RTOG Head and Neck Protocols ITC Submission Checklist  

INITIAL DATA SUBMISSION CHECKLIST
(All initial data is due within one week of the initiation of treatments)

	Digital patient data (CT's and contours)
	Digital planning data (Beams and doses for sum of all treated fields, beams are not required for IMRT delivery)
	Digital planning data (DVHs for total dose plan)
	Film copies of orthogonal simulator films and first day isocenter localization portal radiographs (see QA Guidelines for film requirements). Films may be substituted with digital images in either RTOG or DICOM 3.0 format. Contact ITC if you have questions.
	Hard copy isodoses, axial, sagittal and coronal through isocenter for total dose plan in absolute dose (Gy or cGy)
	Digital Data Submission Information form (see http://atc.wustl.edu/forms/DDSI/ddsi.html)
	Email to itc@wustl.edu AFTER all digital data have been sent

FINAL DATA SUBMISSION CHECKLIST
(All final data is due within one week of the completion of treatments)

	T1 form (copy to ITC, copy to RTOG headquarters)
	copy of daily dose record
